Did this ever get resolved ? I don't 100% this should be the case because :
 
62.136.22.179 does match modem-179.vanadium.dialup.pol.co.uk

The HELO should be discarded in this case. I actually decided to implement
the policy mentioned above at a former workplace. It works at cutting down
on some of the spam (which is good, 'cause the box provides vhosting to the
foolish general public), but has caught a couple of people out. A glance
through the logs found friendsreunited.co.uk mail being returned because
forward lookups did not match the given reverse !
